Nurse to Chaplain: How our Interdisciplinary Team Works to provide Compassionate Care to Those in Need

In hospice, care is delivered through a coordinated group of professionals who work together to address the medical, emotional, spiritual, and practical needs of patients and their families. Here at Gulfside, we have a compassionate team here to help you with every step of the way through your journey. From Chaplains and Social Workers to Certified Nursing Assistants and Physicians, Gulfside is here to support you during life’s most challenging moments. Your care needs are met every step of the way with team members that can assist.

Gulfside Hospice Physician/ Nurse Practitioners

Gulfside’s Hospice Physician provides medical oversight and guidance for each patient’s plan of care. Nurse Practitioners (NPs) are advanced practice registered nurses who asses, diagnose, treat, and manage a patient’s health. Gulfside’s entire care team meets regularly to discuss care and plan for each individual patient. The main responsibilities of Hospice Physicians and Nurse Practitioners are to provide treatment options, prevent complications, manage pain and symptoms, and monitor any changes in illnesses.

Hospice Nurses:

Coordinating care plans with physicians, our skilled nurses provide information about medication, symptoms, pain management, disease concerns, equipment used, and other concerns regarding care. The central responsibilities of Hospice Nurses include pain management, symptom control, patient assessment, and providing training on disease control and prevention.

Hospice Aides/ Certified Nursing Assistants (CNAs)

For our CNAs/hospice aides, providing personal care assistance is their main goal. Under Registered Nurse supervision, the CNAs/hospice aides complete a variety of tasks, including grooming, dressing, bathing, oral hygiene, and positioning. CNAs will also take patients out for walks or wheelchair rides that may be for a primary caregiver without help.

Social Workers/ Emotional Support Caregivers:

Gulfside’s team of specially trained Social Workers provide emotional support and counseling to patients and families during times of need. Hospice care can be the most difficult time of a family’s life. At Gulfside, our team can link you to support services/ community resources, provide support and coping and assist with more advanced directives.

Chaplain / Spiritual Care Support:

Whatever faith or religious denomination, Gulfside’s chaplains are here to provide support to you and your family in times of uncertainty. Chaplains can offer services sensitive to the needs of patients and families, or simply a listening ear or shoulder to lean on.

Volunteer Services:

At Gulfside, our Volunteer Services provide companionship and emotional presence for patients and families in our care. Volunteers assist with respite for caregivers and offer non-clinical support such as reading, conversations, and errands. By contributing their time, skills, and empathy, volunteers can make a meaningful difference in the lives of our patients and families.
